Implementing “Dual-Card Accountability” for Full Safety Compliance

To further enhance intrinsic safety management, Huayang Chemical convened a special safety meeting for all supervisors and above-level managers on April 1, 2019, chaired by General Manager Xu Minghua.  

The meeting began with a report by Wu Jianyou, Director of the EHS Department, on current safety management issues: discrepancies between safety documentation and actual operations, inadequate guidance for employee safety behaviors, and recurring violations of safety protocols.  

Deputy General Manager Zhang Wei, overseeing safety operations, then conveyed higher-level meeting directives. He emphasized that all employees must implement safety responsibility systems, strengthen foundational safety management, prevent major accidents, and reduce minor incidents. Focusing on “two key areas and one major risk,” the company will execute the “One Responsibility, Two Cards” initiative, comprehensively disseminate MSDS for hazardous chemicals and safety operating procedures, and intensify on-site supervision of eight critical work permits. Safety personnel were urged to enhance professional knowledge and technical capabilities to ensure smooth operations.  

In his concluding remarks, GM Xu stressed the meeting’s critical importance and called for unified understanding of safety issues. He outlined five key requirements for April’s Safety Month campaign:  


1. One-Month Safety Rectification  

   - The EHS and Equipment Departments will conduct comprehensive facility inspections to assess operational safety status.  

   - An evaluation report must be completed within one week, with corrective measures for identified gaps.  


2. Enhanced Safety Training  

   - Mandatory training covering:  

      Position-specific safety responsibilities and procedures  

      Hazardous chemical safety  

      Emergency response and fire drills  

   - Managers will receive expert-led training.  

   - Strict testing (90% passing score) with consequences for failures, including withheld training wages and potential termination.  


3. Emergency Preparedness  

   - Quarterly drills for specialized and on-site contingency plans.  

   - Post-incident reviews to refine response protocols.  


4. Safety Responsibility System  

   - Establishing clear safety duties for all levels under the principle: “Safety is everyone’s responsibility.”  


5. Post-Rectification Risk Assessment  

   - Comprehensive evaluation of hazard controls and risk mitigation effectiveness.  


Immediate Actions Taken:  

- Equipment risk assessment completed by April 10.  

- Company-wide signing of safety responsibility agreements initiated.  

- Safety procedure updates and hazardous chemical MSDS compilation underway.  

- Full staff safety training and testing launched April 15.
